Salmolin 200 Rotacap is an inhalation capsule containing Salbutamol (as Salbutamol Sulphate BP) 200 mcg. It is manufactured by The ACME Laboratories Ltd. in Bangladesh.

Indications:

Salmolin 200 Rotacap contains Salbutamol, which is a short-acting beta2-adrenergic agonist. It is commonly used as a "reliever" medication for the rapid relief of symptoms associated with:

  • Asthma: For the treatment and prevention of bronchospasm (tightening of airways) in patients with reversible obstructive airway disease. This includes acute episodes of bronchospasm.
  • Chronic Obstructive Pulmonary Disease (COPD): For the relief of bronchospasm.
  • Exercise-induced bronchospasm (EIB): To prevent bronchospasm triggered by physical exertion.

Pharmacology:

Salbutamol works by stimulating beta2-adrenergic receptors primarily in the smooth muscles of the bronchial airways. This stimulation leads to the relaxation of these muscles, resulting in bronchodilation (widening of the airways) and easier breathing. Its effect is rapid in onset (within minutes) and lasts for about 4-6 hours.

Dosage and Administration:

Salmolin 200 Rotacap is for inhalation only using a Rotahaler device. The capsule should not be swallowed.

  • For relief of acute asthma symptoms (bronchospasm):
    • Adults and adolescents (12 years and older): One or two inhalations (200-400 mcg) as required.
    • Children (6-11 years): One inhalation (200 mcg) as required.
  • For prevention of exercise-induced bronchospasm:
    • Adults and adolescents (12 years and older): Two inhalations (400 mcg) 10-15 minutes before exercise.
    • Children (6-11 years): One inhalation (200 mcg) 10-15 minutes before exercise.
  • For long-term maintenance treatment (regular use):
    • Adults and adolescents (12 years and older): Up to two inhalations (400 mcg) four times a day.
    • Children (6-11 years): Up to one inhalation (200 mcg) four times a day.

Important points for administration:

  • Patients should be instructed on the proper use of the Rotahaler device.
  • The capsule should be inserted into the Rotahaler immediately before use.
  • Do not exceed the prescribed dose. Increasing the dose or frequency of Salbutamol without medical advice may indicate worsening asthma control.

Side Effects:

Side effects are generally mild and dose-dependent. Common side effects include:

  • Tremor: Especially in the hands, can be more pronounced at higher doses.
  • Headache
  • Tachycardia: Increased heart rate, palpitations.
  • Muscle cramps
  • Nervousness or restlessness
  • Oropharyngeal irritation (sore throat, cough)

Less common or rare side effects can include:

  • Paradoxical bronchospasm (sudden worsening of breathing after inhalation) – if this occurs, discontinue immediately and use an alternative bronchodilator.
  • Hypersensitivity reactions (rash, angioedema, urticaria, bronchospasm, hypotension, collapse)
  • Hypokalemia (low potassium levels, especially with high doses or in severe asthma)
  • Arrhythmias (irregular heart rhythms)
  • Myocardial ischemia (heart muscle pain)
  • Hyperactivity in children

Contraindications:

Salmolin 200 Rotacap is contraindicated in patients with:

  • Known hypersensitivity to Salbutamol or any component of the formulation.

Precautions and Warnings:

  • Not for Routine Daily Use: Salbutamol should be used on an "as needed" basis for symptom relief. If a patient finds their need for Salmolin increasing, it may indicate deteriorating asthma control, and a medical review is necessary to consider increasing regular maintenance therapy (e.g., inhaled corticosteroids).
  • Cardiovascular Disorders: Use with caution in patients with pre-existing cardiovascular disorders (e.g., coronary insufficiency, arrhythmias, hypertension), as beta2-agonists can affect heart rate and blood pressure.
  • Diabetes Mellitus: Beta2-agonists can cause transient increases in blood glucose levels. Monitor blood glucose in diabetic patients.
  • Thyrotoxicosis: Use with caution in patients with hyperthyroidism.
  • Hypokalemia: Serious hypokalemia may result from beta2-agonist therapy, especially in severe acute asthma.
  • Seizure Disorders: Use with caution in patients with seizure disorders.
  • Paradoxical Bronchospasm: If this occurs, discontinue the product immediately and institute alternative therapy.

Pregnancy and Lactation:

  • Pregnancy (US FDA Pregnancy Category C): Salbutamol should be used during pregnancy only if the potential benefit outweighs the potential risk to the fetus. 
  • Lactation:It is not known whether Salbutamol is excreted in human breast milk. Caution should be exercised when Salbutamol is administered to a nursing mother.

Drug Interactions:

  • Beta-blockers: Both non-selective and selective beta-blockers (e.g., Propranolol) should generally be avoided in patients with asthma, as they can block the bronchodilator effect of Salbutamol and cause severe bronchospasm.
  • Diuretics: Non-potassium-sparing diuretics (e.g., loop or thiazide diuretics) can worsen hypokalemia caused by beta2-agonists.
  • Digoxin: Rarely, high doses of Salbutamol may cause a transient decrease in digoxin levels.
  • Tricyclic Antidepressants (TCAs) and Monoamine Oxidase Inhibitors (MAOIs): Concurrent administration may potentiate the cardiovascular effects of Salbutamol.
  • Other Sympathomimetics: Avoid concomitant use with other sympathomimetic agents due to potential additive effects on the cardiovascular system.

Storage Conditions:

Store below 30°C in a dry place, protected from light. Keep out of the reach of children. The capsules should only be inserted into the Rotahaler device immediately prior to use.
