Salost DR (Risedronate Sodium): A Bisphosphonate for Bone Health
Salost DR contains Risedronate Sodium, which belongs to a group of medicines called bisphosphonates. It works by altering the cycle of bone formation and breakdown, slowing bone loss and increasing bone mass, thereby helping to prevent bone fractures. It is indicated for preventing and treating osteoporosis and Paget's disease. It is classified under Bisphosphonate preparations.
How Salost DR Works (Pharmacology)
- Bone Remodeling: Your body constantly undergoes a process of breaking down old bone (resorption) and forming new bone tissue. In conditions like osteoporosis, the rate of bone breakdown exceeds bone formation, leading to gradual bone loss, making bones thinner and fragile.
- Antiresorptive Action: Risedronate has a strong affinity for hydroxyapatite crystals, which are a major component of bone.
- Inhibits Osteoclasts: At the cellular level, Risedronate specifically inhibits osteoclasts. Osteoclasts are the cells responsible for bone resorption. While they still adhere to the bone surface, their active resorption process is significantly reduced. This slowing of bone breakdown allows new bone formation to catch up, leading to an increase in bone mass and density, thereby reducing fracture risk.
Key Indications & Benefits
Salost DR is indicated for:
- Prevention and treatment of osteoporosis in postmenopausal women.
- Increasing bone mass in men with osteoporosis.
- Prevention and treatment of osteoporosis in men and women caused by treatment with steroid medicines (e.g., prednisone).
- Treatment of Paget’s disease of bone in men and women.
Dosage & Administration
The absorption of Risedronate Sodium is affected by food. Therefore, it is crucial to follow specific administration instructions. Always consult a registered physician for precise dosage instructions.
General Administration Instructions:
- Take Risedronate tablets at least 30 minutes before the first food, other medicinal product, or drink (other than plain water) of the day to ensure adequate absorption.
- The tablet must be swallowed whole and not sucked or chewed.
- To aid delivery to the stomach, take the tablet while in an upright position with a glass of plain water (120 ml or more).
- Do not lie down for 30 minutes after taking the tablet.
- Supplemental calcium and vitamin D should be considered if dietary intake is inadequate.
Specific Dosages:
- Treatment of postmenopausal osteoporosis (to reduce vertebral and hip fracture risk):
- Risedronate Sodium 5 mg once daily.
- OR Risedronate Sodium 35 mg once weekly on the same day each week.
- OR Risedronate Sodium 150 mg once monthly.
- Prevention of osteoporosis in postmenopausal women with increased risk:
- Risedronate Sodium 5 mg once daily.
- To maintain or increase bone mass in postmenopausal women undergoing long-term systemic corticosteroid treatment (doses ≥ 7.5 mg/day prednisone or equivalent for more than 3 months):
- Risedronate Sodium 5 mg once daily.
- Treatment of osteoporosis in men at high risk of fractures:
- Risedronate Sodium 35 mg once weekly, taken on the same day each week.
Missed Doses:
- Risedronate 5 mg tablet: If an occasional dose is missed, it can be taken before breakfast, between meals, or in the evening, adhering to the empty stomach instructions.
- Risedronate 35 mg tablet: If a dose is missed, take one tablet the morning after remembering. Then, return to the regular once-a-week schedule. Do not take two tablets on the same day.
- Risedronate 150 mg tablet: If a dose is missed and the next month's scheduled dose is more than 7 days away, take the missed tablet the morning after remembering. Then, return to the regular once-a-month schedule.

Contraindications:
- Known hypersensitivity to Risedronate Sodium.
- Hypocalcemia (low blood calcium).
- Severe renal impairment (creatinine clearance lower than 30 ml/min).

Side Effects:
- Serious side effects (seek immediate medical attention if experienced): Chest pain; difficulty or pain when swallowing; pain or burning under the ribs or in the back; new or worsening heartburn; severe joint, bone, or muscle pain; jaw pain, numbness, or swelling.
- Less serious side effects: Mild heartburn or stomach upset; diarrhea, gas, or constipation; mild joint or back pain; headache.
Pregnancy & Lactation:
- Salost DR is intended for use only in postmenopausal women.
- Pregnancy: Should not be taken during pregnancy. If taken by accident, stop immediately and consult a doctor. We do not know if it can harm an unborn child.
- Breast-feeding: Should not be taken by breast-feeding women. If taken by accident, stop immediately and consult a doctor. We do not know if it can pass through milk and harm the baby.
Precautions & Warnings:
- Hypocalcemia: Do not take if you have low blood calcium. Correct hypocalcemia before starting Salost DR.
- Upright Position: You must be able to sit or stand upright for at least 30 minutes after taking the tablet.
- Renal Impairment: Use with caution in patients with poorly functioning kidneys. Contraindicated in severe renal impairment (CrCl < 30 ml/min).
- Allergy: Do not take if you have a known allergy to Salost DR. If an allergic reaction (e.g., swelling of face/tongue/throat, difficulty breathing/swallowing, skin rash) occurs during treatment, stop immediately and seek medical advice. Do not restart therapy if hypersensitivity reactions occurred.
- Children: Not intended for use in children.
Use in Special Populations:
- Pediatric use: Safety and efficacy have not been established in children and adolescents.
- Elderly: No dosage adjustment is necessary for elderly patients (≥ 60 years), including those ≥ 75 years, as bioavailability, distribution, and elimination are similar to younger subjects.
- Renal impairment: No dosage adjustment for mild to moderate renal impairment. Contraindicated in severe renal impairment (CrCl < 30 ml/min).
Drug Interactions:
- Aspirin or other NSAIDs: Inform your doctor if taking these.
- Antacids, supplements, or medicines containing aluminum, calcium, magnesium, or other minerals: These can interfere with Salost DR absorption. Do not take them for at least 30 minutes after taking a Salost DR tablet.
Overdose Effects
- No specific information on overdose treatment.
- Expected effects include decreases in serum calcium, and signs and symptoms of hypocalcemia may occur.
- Treatment: Give milk or antacids containing magnesium, calcium, or aluminum to bind Salost DR and reduce absorption. In substantial overdose, gastric lavage may be considered to remove unabsorbed medicine.
- Hemodialysis: Does not significantly enhance clearance of Salost DR.
