Seclo (Omeprazole): A Proton Pump Inhibitor (PPI)

Seclo contains Omeprazole, a substituted benzimidazole and a potent inhibitor of gastric acid secretion. It is a prescription medicine used to treat and prevent a variety of acid-related conditions, including ulcers, reflux disease, and Zollinger-Ellison syndrome. It belongs to the therapeutic class of Proton Pump Inhibitors.


How Seclo Works (Pharmacology)

Omeprazole's mechanism of action is as follows:

  • Inhibits Gastric Acid Secretion: It inhibits the hydrogen-potassium-adenosine triphosphatase (H+/K+ ATPase) enzyme system, also known as the "proton pump," located in the gastric parietal cells of the stomach.
  • Blocks the Final Step: By blocking this pump, Omeprazole effectively prevents the final step of acid production, leading to a significant and long-lasting reduction in gastric acid secretion.
  • Onset and Duration: The antisecretory effect begins within one hour of oral administration, with the maximum effect occurring within two hours. The inhibition of acid secretion can last for up to 72 hours. When the drug is discontinued, secretory activity returns gradually over 3 to 5 days.

Key Indications & Benefits

Seclo is indicated for the treatment of:

  • Gastric and duodenal ulcers.
  • NSAID-associated ulcers (treatment and prophylaxis).

Dosage & Administration

Seclo can be administered orally (as capsules or tablets) or via IV injection/infusion, depending on the condition. Always consult a registered physician for specific dosage instructions.

Oral Dosage:

  • Benign gastric and duodenal ulcer: 20 mg once daily.
  • NSAID-associated ulcers: 20 mg once daily.
  • GERD: 20 mg once daily for 4 weeks.
  • Long-term management of acid reflux: 10-20 mg daily.
  • Acid-related dyspepsia: 10-20 mg once daily for 2-4 weeks.
  • Prophylaxis of acid aspiration: 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery.
  • Zollinger-Ellison syndrome: Initially 60 mg once daily; usual range 20-120 mg daily. (Doses > 80 mg should be divided).
  • H. pylori eradication: 20 mg twice daily, in combination with specific antimicrobial agents.
  • Pediatric use (Child > 1 year) for severe ulcerating reflux esophagitis:
    • Body-weight 10-20 kg: 10-20 mg once daily.
    • Body-weight > 20 kg: 20-40 mg once daily.

IV Injection/Infusion:

  • Prophylaxis of acid aspiration: 40 mg as a slow IV injection one hour before surgery.
  • Ulcers or reflux esophagitis (when oral medication is inappropriate): 40 mg IV once daily.
  • Zollinger-Ellison Syndrome (ZES): Initial dose 60 mg IV daily.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before taking Seclo.

Contraindications:

  • Known hypersensitivity to omeprazole.
  • The possibility of gastric malignancy should be excluded before starting treatment for a gastric ulcer, as omeprazole may mask symptoms and delay diagnosis.

Side Effects:

  • Seclo is generally well tolerated, and adverse reactions are typically mild and reversible.
  • Gastrointestinal: Diarrhea, headache, constipation, nausea/vomiting, flatulence, abdominal pain.
  • Skin: Rash, urticaria, pruritus (usually resolve upon discontinuation). In isolated cases: photosensitivity, bullous eruption, erythema multiforme, angioedema, alopecia.
  • Neurological/Psychiatric: Dizziness, lightheadedness, somnolence, insomnia, vertigo. Rarely: reversible mental confusion, agitation, depression, and hallucinations (predominantly in severely ill patients).
  • Other (isolated cases): Arthritic and myalgic symptoms, blurred vision, taste disturbance, peripheral edema, increased sweating, gynecomastia, blood disorders (leucopenia, thrombocytopenia, etc.), anaphylactic shock, fever, bronchospasm, and liver/renal issues.

Pregnancy & Lactation:

  • Pregnancy: Omeprazole can be used during pregnancy as three prospective studies indicate no adverse effects on pregnancy or the fetus.
  • Lactation: No information is available on the passage of omeprazole into breast milk. If the use of omeprazole is considered essential, breast-feeding should be discontinued.

Precautions & Warnings:

  • Clopidogrel: Avoid concomitant use as it may reduce the pharmacological activity of clopidogrel.
  • Osteoporosis: Observational studies suggest a possible increased risk for osteoporosis-related fractures of the hip, wrist, or spine with long-term PPI therapy.
  • Atrophic Gastritis: Occasionally noted in long-term users.
  • Methotrexate: Concomitant use with PPIs may lead to methotrexate toxicities.
  • Children and Adolescents: Safety and effectiveness have not been established in patients younger than 18 years of age.

Drug Interactions:

  • Ketoconazole: Absorption may be reduced due to decreased stomach acidity.
  • Diazepam, Phenytoin, Warfarin: Omeprazole is metabolized by cytochrome P450 and can delay the elimination of these drugs. Monitoring is recommended, and dose adjustments may be necessary.
  • Clarithromycin: Plasma concentrations of both drugs are increased when taken together, which is a useful interaction for H. pylori eradication.
  • Digoxin: Simultaneous use may lead to a 10% increase in digoxin bioavailability.
  • Food or alcohol: Do not affect the absorption of Omeprazole.
