Indications:Secrin M XR is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both glimepiride and metformin is appropriate.It is used in patients whose blood sugar is not adequately controlled with diet and exercise alone or with monotherapy of either glimepiride or metformin.

Dosage and Administration: The initial recommended dose is one tablet of Secrin M 1/500 XR once daily with breakfast or the first main meal of the day. The dosage should be individualized based on the patient's response and tolerability, and the current treatment regimen. For patients inadequately controlled on glimepiride or metformin monotherapy, the starting dose is Secrin M 1/500 XR once daily, and it should be gradually titrated after assessing the therapeutic response. The daily dose can be increased in increments of 1 tablet every 1-2 weeks based on blood glucose monitoring. The maximum recommended daily dose is glimepiride 8 mg and metformin 2000 mg.Due to the extended-release formulation, the tablet must be swallowed whole and not crushed or chewed.  

Side Effects: The most common side effect is hypoglycemia (low blood sugar), especially when used with other antidiabetic agents or if meals are skipped. Symptoms may include sweating, paleness, tremor, anxiety, unusual tiredness or weakness, confusion, difficulty concentrating, hunger, and visual disturbances. Gastrointestinal side effects associated with metformin, such as nausea, vomiting, diarrhea, and abdominal discomfort, may also occur, particularly at the start of treatment.Other possible side effects include headache and taste disturbances.

Precautions:Secrin M XR is contraindicated in patients with type 1 diabetes mellitus, diabetic ketoacidosis, severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis, and known hypersensitivity to glimepiride, metformin, other sulfonylureas, or other biguanides.It should be used with caution in patients with impaired hepatic function and in elderly or debilitated patients who may be more susceptible to hypoglycemia. Alcohol consumption should be avoided as it can increase the risk of hypoglycemia and lactic acidosis (a rare but serious metabolic complication associated with metformin). Renal function should be assessed before initiating treatment and regularly thereafter. Temporary discontinuation of metformin may be necessary for surgical procedures or iodinated contrast studies.
