Siglimet 50/500 

Presentation: Siglimet 50/500 Tablet, available in Bangladesh, is a combination tablet containing Sitagliptin 50 mg and Metformin Hydrochloride 500 mg. 

Indications: Siglimet is indicated in adults with type 2 diabetes mellitus as an adjunct to diet and exercise to improve glycemic control when treatment with both sitagliptin and metformin is appropriate.

Dosage and Administration: The dosage of Siglimet should be individualized based on the patient's current regimen, effectiveness, and tolerability, without exceeding the maximum recommended daily dose of sitagliptin 100 mg and metformin 2000 mg. Siglimet 50/500 is usually administered twice daily with meals to reduce the gastrointestinal side effects associated with metformin.

• The starting dose should be based on the patient's current antidiabetic regimen.
• For patients not currently treated with metformin, the recommended starting dose is 50 mg sitagliptin/500 mg metformin hydrochloride twice daily, with gradual dose escalation recommended to reducegastrointestinal side effects associated withmetformin.
• For patients switching from separate tablets of sitagliptin and metformin, Siglimet should be initiated at the same total daily doses of each component.
• Renal function should be assessed before initiating Siglimet and periodically thereafter. Siglimet is not recommended in patients with moderate or severe renal impairment (eGFR < 45 mL/min/1.73 m²).

Side Effects: Common side effects may include upper respiratory tract infection, headache, diarrhea, nausea, and abdominal discomfort. Hypoglycemia may occur when Siglimet is used in combination with insulin or sulfonylureas.

Precautions: Siglimet is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), or a history of serious hypersensitivity reaction to sitagliptin or metformin. It is not recommended for use in patients with type 1 diabetes mellitus. Caution should be exercised in patients with moderate renal impairment, a history of pancreatitis, heart failure, and excessive alcohol intake due to the risk of lactic acidosis associated with metformin.

Storage: Store Siglimet 50/500 tablets below 30°C, protected from light and moisture. Keep out of reach of children.
