Sliptin-M 1000 

Presentation:Sliptin-M 1000 is a tablet formulation containing two active pharmaceutical ingredients: 50 mg of Sitagliptin and Metformin Hydrochloride BP 1000 mg.It is presented as a film-coated tablet and is available in blister packs.  

Indications:Sliptin-M 1000 is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both sitagliptin and metformin is appropriate.This combination is used in patients whose blood sugar is not adequately controlled with metformin alone or who are already treated with sitagliptin and metformin as separate tablets.

Dosage and Administration:The dosage of Sliptin-M 1000 should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommendeddaily dose of 100 mg of sitagliptin and 2000 mg of metformin.It is generally taken twice daily with meals to reduce the gastrointestinal side effects associated with metformin.

• For patients not currently on metformin, the starting dose is typically 50 mg sitagliptin/500 mg metformin twice daily, with gradual dose escalation as needed. 
• For patients already on metformin, the recommended starting dose is 50 mg sitagliptin/1000 mg metformin twice daily.
• Patients taking metformin 1000 mg twice daily may be switched to Sliptin-M 50 mg/1000 mg twice daily.

Renal function should be assessed prior to initiation and periodically thereafter.Sliptin-M 1000 is contraindicated in patients with an eGFR below 45 mL/min/1.73 m².

Side Effects: Common side effects associated with Sliptin-M 1000 may include diarrhea, upper respiratory tract infections, headache, and nasopharyngitis. Hypoglycemia can occur when used in combination with insulin or sulfonylureas. Gastrointestinal effects such as nausea, vomiting, and abdominal discomfort are also common, particularly during metformin initiation. Pancreatitis has been reported with sitagliptin. Lactic acidosis is a rare but serious potential side effect associated with metformin.

Precautions: Sliptin-M 1000 is contraindicated in patients with severe renal impairment (eGFR < 45 mL/min/1.73 m²), metabolic acidosis (including diabetic ketoacidosis), or known hypersensitivity to sitagliptin or metformin. It is not recommended for use in patients with type 1 diabetes. Caution should be exercised in patients with risk factors for lactic acidosis, a history of pancreatitis, and heart failure. Renal function should be monitored regularly.

Storage: Store Sliptin-M 1000 tablets below 30°C in a dry place, protected from light and moisture. Keep out of the reach of children. Do not use after the expiry date stated on the packaging.
