Spilac-F (Frusemide & Spironolactone): A Combined Diuretic

 

Spilac-F is a combination of two diuretics: Frusemide (a loop diuretic) and Spironolactone (a potassium-sparing diuretic). This combination provides a complementary and synergistic diuretic effect. It is indicated for the treatment of essential hypertension, chronic congestive heart failure, hepatic cirrhosis with ascites, and edema. It is also used for hyperaldosteronism and resistant edema associated with secondary hyperaldosteronism. Spilac-F belongs to the therapeutic class of potassium-sparing diuretics and aldosterone antagonists.


 

How Spilac-F Works (Pharmacology)

 

The two components work together to provide a powerful diuretic effect while minimizing potassium loss:

  • Frusemide (Loop Diuretic): It inhibits the Na+/K+/2Cl- co-transporter in the ascending Loop of Henle. This blocks the reabsorption of sodium, potassium, and chloride, leading to a significant increase in the excretion of sodium and water. This action typically causes a loss of potassium.
  • Spironolactone (Potassium-Sparing Diuretic): It acts at the distal tubule to antagonize the action of aldosterone. This promotes the excretion of sodium in exchange for potassium, which helps to counteract the potassium loss induced by Frusemide.
  • Synergistic Effect: The combined action of the two drugs results in a more potent diuretic effect than either drug alone, with the benefit of reducing the risk of hypokalemia.

Dosage & Administration

 

The dosage of Spilac-F is based on the patient's response and varies depending on the strength of the tablet. It is not suitable for use in children. Always consult a registered physician for specific dosage instructions.

  • Frusemide 20 mg & Spironolactone 50 mg: 1 to 4 tablets daily.
  • Frusemide 40 mg & Spironolactone 50 mg: 1 to 2 tablets daily, for patients who are already stabilized on a higher dosage.
  • Elderly patients: May excrete the drugs more slowly, so caution is advised.

 

Important Considerations & Warnings

Contraindications:

  • Anuria (absence of urine production).
  • Acute renal insufficiency or severe impairment of renal function (creatinine clearance <30 ml/min).
  • Hyperkalemia (high potassium levels).
  • Addison's disease.
  • Hypersensitivity to Spironolactone, Frusemide, or sulfonamides.

Side Effects:

  • Spironolactone-related: Headache, drowsiness, gastrointestinal distress (cramp, diarrhea), skin rashes, and endocrine disorders like gynecomastia, hirsutism, and menstrual irregularities.
  • General: Excessive diuresis can lead to dehydration, a reduction in blood volume, and the possibility of vascular thrombosis or embolism, particularly in the elderly.
  • Electrolyte Imbalance: It can cause hyponatremia and hyperkalemia. Serious depletion of potassium and magnesium may lead to cardiac arrhythmias.

Pregnancy & Lactation:

  • Pregnancy: Spironolactone and its metabolites can cross the placental barrier. Frusemide may cause fetal abnormalities. The use of this drug in women of childbearing age or pregnant women should only be considered if the benefits outweigh the risks and with the use of appropriate contraceptive measures.
  • Lactation: Both Spironolactone and Frusemide are excreted in breast milk. If the use of this medication is essential, breastfeeding should be discontinued.

Precautions & Warnings:

  • Electrolyte Imbalance: Caution should be taken in patients prone to electrolyte deficiency.
  • Other Conditions: Use with caution in patients with diabetes, enlarged prostate, hypotension, and hypovolemia.

Drug Interactions:

  • ACE inhibitors or potassium salts: Increased risk of hyperkalemia.
  • Cardiac glycosides (e.g., digoxin): Spironolactone can increase blood levels of digoxin, leading to toxicity.
  • NSAIDs (e.g., indomethacin): May reduce or abolish the blood pressure-lowering and diuretic effects of Frusemide.
  • Aminoglycoside antibiotics: Frusemide may increase their ototoxicity.
