Sudac (Sulindac): A Non-Steroidal Anti-inflammatory Drug (NSAID)

Sudac contains Sulindac, a non-steroidal antirheumatic agent with anti-inflammatory, analgesic, and anti-pyretic properties. It is indicated for the symptomatic treatment of various inflammatory and painful conditions, including arthritis, periarticular disorders, and gouty arthritis. It belongs to the therapeutic class of NSAIDs, as well as drugs for Osteoarthritis, Rheumatoid Arthritis, and Gout.


How Sudac Works (Pharmacology)

Sulindac's mechanism of action is related to its inhibition of prostaglandin synthesis:

  • Prodrug Activation: After absorption, Sulindac is a prodrug that is reversibly reduced to its active form, the sulphide metabolite, in the body.
  • Inhibits Prostaglandin Synthesis: The sulphide metabolite is a potent inhibitor of prostaglandin synthesis. Prostaglandins are lipid compounds that mediate inflammation, pain, and fever.
  • Anti-inflammatory Effects: By inhibiting prostaglandin synthesis, Sulindac reduces inflammation, pain, and swelling associated with various conditions.

Sudac is indicated for the symptomatic treatment of:

  • Rheumatoid arthritis

Dosage & Administration

Sudac should be administered orally twice a day with food. The dosage should be adjusted based on the patient's response, and the maximum dosage is 400 mg per day. Dosages above 400 mg per day are not recommended. Always consult a registered physician for specific dosage instructions.

  • Osteoarthritis, Rheumatoid Arthritis, & Ankylosing Spondylitis:
    • Recommended starting dosage is 150 mg twice a day.
  • Acute Painful Shoulder & Acute Gouty Arthritis:
    • Recommended dosage is 200 mg twice a day.
  • Dosage Adjustment: After a satisfactory response is achieved, the dosage may be reduced.

Contraindications:

  • Patients with a known allergy to Sulindac.
  • Individuals in whom acute asthmatic attacks, urticaria, or rhinitis have been precipitated by aspirin or other NSAIDs.
  • Patients with a history of active gastrointestinal bleeding or peptic ulceration.
  • Children, pregnant, or lactating women.

Side Effects:

  • Gastrointestinal: Abdominal pain, nausea, and constipation are most common. Gastrointestinal ulceration and bleeding can also occur.
  • Central Nervous System: Drowsiness, dizziness, headache, and nervousness are frequent. Other effects include depression, tinnitus, confusion, and insomnia.
  • Other: Edema, weight gain, hypertension, hematuria, skin rashes, pruritus, hypersensitivity reactions (a syndrome of fever, rashes, hepatotoxicity, renal toxicity, and blood disorders), hepatitis, jaundice, or renal failure.

Pregnancy & Lactation:

  • Pregnancy: Not recommended for use during the third trimester. During the first two trimesters, it should only be used if the potential benefit justifies the potential risk to the fetus.
  • Lactation: It is not known whether Sulindac is excreted in human milk. A decision should be made to either discontinue nursing or discontinue the medication, taking into account the importance of the medicine to the mother.

Precautions & Warnings:

  • Caution: Administer with caution to patients with impaired renal function, bleeding disorders, epilepsy, parkinsonism, or psychiatric disorders.
  • Hepatic Impairment: The half-life of Sudac is prolonged in patients with impaired liver function, and a reduction in daily dosage may be required.
  • Anemia: Anemia is sometimes seen in patients receiving NSAIDs, including Sudac.
  • Concomitant Medications: Monitor patients on anticoagulants or hypoglycemic agents carefully, as dosage adjustments may be needed. Avoid prolonged concurrent use with paracetamol due to the increased risk of adverse renal effects.

Drug Interactions:

  • Aspirin: Decreases the bioavailability of Sulindac's active metabolite.
  • Paracetamol: Prolonged concurrent use may increase the risk of adverse renal effects.
  • Probenecid: May increase plasma concentrations of Sulindac and its inactive metabolite while slightly decreasing the active metabolite's concentration.
  • Anticoagulants and Hypoglycemic Agents: Careful monitoring is required, and dosage adjustments may be necessary.

Overdose Effects

  • Symptoms: Generally reflect the gastrointestinal, renal, and central nervous system toxicities of Sudac.
  • Treatment: Treatment is symptomatic and supportive.
