Presentation: Each film-coated tablet of Taglimet 1000 contains 50 mg of sitagliptin (as sitagliptin phosphate monohydrate) and 1000 mg of metformin hydrochloride.
Indications:Taglimet is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both sitagliptin and metformin is appropriate.
Dosage and Administration: The usual dose is one tablet twice daily with meals, with gradual dose escalation to reduce gastrointestinal side effects associated with metformin. Thedosage should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommendeddaily dose of 100 mg sitagliptin and 2000 mg metformin. For patients already on metformin 850 mg twice daily, the recommended starting dose of Taglimet is one tablet twice daily.
Side Effects: Common side effects may include diarrhea, upper respiratory tract infection, headache, nausea, and hypoglycemia (when used in combination with insulin or sulfonylureas). Metformin component may cause gastrointestinal disturbances such as nausea, vomiting, and abdominal discomfort, particularly at the start of treatment. Lactic acidosis is a rare but serious potential side effect associated with metformin.
Precautions: Taglimet is contraindicated in patients with hypersensitivity to sitagliptin, metformin, or any of the excipients, severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis, including diabetic ketoacidosis, and conditions associated with tissue hypoxia. Caution should be exercised in patients with moderate renal impairment, heart failure, and hepatic impairment. Alcohol consumption should be avoided. Renal function should be assessed before initiating Taglimet and regularly thereafter.
Storage: Store in a cool and dry place, protected from light and moisture. Keep out of reach of children.
