Taglimet 500

Presentation:Each tablet of Taglimet 500 contains 500 mg of metformin hydrochloride and 50 mg of sitagliptin.

Indications:Taglimet is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhen treatment with both sitagliptin and metforminis appropriate.It is used in patients whose blood sugar is not controlled well enough with metformin alone or who are already taking sitagliptin and metformin as separate tablets.

Dosage and Administration:The dosage of Taglimet should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommendeddaily dose of 100 mg of sitagliptin and 2000 mg of metformin.It is usually taken twice daily with meals to reduce the gastrointestinal side effects associated with metformin.

• For patients not currently treated with metformin, the starting dose is usually 50 mg sitagliptin/500 mg metformin hydrochloride twice daily.  
• For patients already treated with metformin, the starting dose of Taglimet should provide the sitagliptin dose of 50 mg twice daily, and the metformin dose should be similar to the dose already being taken.Renal function should be assessed before initiating Taglimet and periodically thereafter. Taglimet is contraindicated in patients with an eGFR below 30 mL/min/1.73 m².  

Side Effects:Common side effects may include upper respiratory tract infection, nasopharyngitis, and headache.Due to the metformin component, gastrointestinal side effects such as nausea, vomiting, diarrhea, and abdominal discomfort may also occur, particularly at the start of treatment.Hypoglycemia may occur when Taglimet is used in combination with insulin or sulfonylureas.Pancreatitis has been reported with sitagliptin.  

Precautions: Taglimet is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute metabolic acidosis, including diabetic ketoacidosis, and known hypersensitivity to sitagliptin, metformin, or any of the excipients. It is not recommended for use in patients with type 1 diabetes mellitus. Assess renal function before initiation and monitor periodically. Avoid excessive alcohol intake. Metformin should be temporarily discontinued in patients undergoing radiologic studies with intravascular administration of iodinated contrast materials or any surgical procedures necessitating restricted intakeof food and fluids.Caution should be exercised in patients with a history of pancreatitis.

Storage: Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
