📋 Indications: Thyroid Replacement & Suppression

Thyronor is indicated for:

• Hypothyroidism: Total hormone replacement therapy for underactive thyroid function of any origin (except temporary hypothyroidism during the recovery phase of subacute thyroiditis).

• TSH Suppression: Lowering Thyroid Stimulating Hormone levels to treat or manage goiters, thyroid nodules, and to manage thyroid cancer recovery following surgical or radiological interventions.

• Drug-Induced Goiters: Suppressing the thyroid-enlarging effects of specific medications like Lithium.

🥄 Dosage & Administration Protocol

• How to Take: For optimal absorption, Levothyroxine must always be taken on an empty stomach first thing in the morning, at least 30 to 60 minutes before breakfast or coffee.

Adult Dosing Guidelines

• Standard Hypothyroidism: Initial starting dose is 25 to 50 mcg once daily. Your physician will adjust this in increments of 12.5 to 25 mcg every 6 to 8 weeks until your blood $TSH$ levels normalize.

• Severe Hypothyroidism: Started lower (12.5 to 25 mcg/day) and increased by 25 mcg/day every 2 to 4 weeks under strict clinical monitoring.

• Patients Over 50 (or with Heart Disease): The target maintenance dose is calculated carefully at 1.7 mcg/kg/day.

Pediatric Dosing (Age-Specific)

Doses are highly sensitive and must be tailored strictly to weight and continuous lab reviews:

• 0 to 3 months: 10–15 mcg/kg/day

• 3 to 6 months: 8–10 mcg/kg/day

• 6 to 12 months: 6–8 mcg/kg/day

• 1 to 5 years: 5–6 mcg/kg/day

• 6 to 12 years: 4–5 mcg/kg/day

• Over 12 years (puberty incomplete): 2–3 mcg/kg/day

• Growth & puberty complete: 1.7 mcg/kg/day

⚠️ Critical Safety Warnings & Cardiac Precautions

• Cardiovascular Flare-ups: If you have underlying heart disease, are over 50, or have long-standing severe hypothyroidism, starting too high or increasing doses too rapidly can cause chest pain (angina), dangerous heart arrhythmias, a sudden spike in blood pressure, or a heart attack. Dosing changes must be small and gradual.

• Adrenal Insufficiency Check: If hypothyroidism is caused by pituitary gland failure, an uncorrected adrenal gland failure may also coexist. Adrenal insufficiency must be completely treated with corticosteroid replacement before starting Thyronor to prevent a life-threatening acute adrenal crisis.

• Weight Fluctuations: Any significant change in your body weight requires a re-evaluation of your thyroid panel, as your required dosage may change.

🛑 Contraindications & Drug Interactions

• Contraindications: Do not use if you have untreated hyperthyroidism (thyrotoxicosis), an uncorrected adrenal insufficiency, or during an acute heart attack (myocardial infarction).

• Diabetes Medication Interaction: Adding Thyronor to insulin or oral antidiabetic treatments can change your body's glucose metabolism, often increasing your insulin or antidiabetic medication requirements. Blood sugar must be monitored closely.

• Antidepressants: Concurrent use with tricyclic or tetracyclic antidepressants can abnormally amplify the therapeutic and toxic side effects of both drugs, elevating the risk of heart arrhythmias.

🤰 Pregnancy & Lactation

• Pregnancy (Category A): Safe and highly essential to continue during pregnancy to support fetal development. Note that pregnancy often increases your body's Levothyroxine requirements. Regular blood tests are mandatory to adjust doses upward as needed.

• Lactation: Only trace amounts enter breast milk. Adequate thyroid replacement is critical for the mother to maintain healthy, normal lactation.
