Tiapine XR 50 Tabletcontains 50 mg of Quetiapine Fumarate in an extended-release formulation.  

Generic Name:Quetiapine Fumarate  

Manufacturer:General Pharmaceuticals Ltd. (in Bangladesh) 

Presentation: Each extended-release tablet contains Quetiapine Fumarate BP equivalent to Quetiapine 50 mg. It is usually available in strips of 10 tablets. The unit price is around ৳ 8.00, and a strip of 10 tablets costs approximately ৳ 80.00. 

Indications: Tiapine XR is an atypical antipsychotic medication used to treat:

  • Schizophrenia in adults and adolescents 13 years and older.
  • Bipolar disorder, including manic episodes, depressive episodes, and maintenance treatment in adults.
  • Major depressive disorder (as an adjunct to other antidepressants).

Dosage and Administration: The dosage varies depending on the condition being treated and the patient's age and response. It is administered orally once daily. Tiapine XR tablets should be swallowed whole and not chewed or crushed. It can be taken with or without food, but consistency regarding food intake (either always with food or always without) is generally recommended. Your doctor will determine the appropriate starting dose and maintenance dose. Some general guidelines:

  • Schizophrenia (Adults): The total daily dose for the first four days is 50 mg (Day 1), 100 mg (Day 2), 200 mg (Day 3), and 300 mg (Day 4). From Day 4 onwards, the dose should be titrated to the usual effective dose range of 300-450 mg/day, which may be adjusted between 150 to 750 mg/day based on individual response and tolerability. 
  • Schizophrenia (Adolescents 13-17 years): The dosing is similar to adults.
  • Bipolar Mania (Adults): The total daily dose for the first four days is 100 mg (Day 1), 200 mg (Day 2), 300 mg (Day 3), and 400 mg (Day 4). Further dosage adjustments up to 800 mg/day by Day 6 should be in increments of no greater than 200 mg/day. The dose may range from 200 to 800 mg/day. 
  • Bipolar Depression (Adults): The usual dose is 300 mg/day, administered once daily at bedtime. The daily dose for the first four days is 50 mg (Day 1), 100 mg (Day 2), 200 mg (Day 3), and 300 mg (Day 4), and can be titrated up to 600 mg by Day 8.
  • Maintenance treatment of Bipolar I Disorder: The dose range is typically 400 to 800 mg/day.
  • Major Depressive Disorder (Adults): The starting dose is usually 50 mg once daily at bedtime, with titration up to 300 mg/day by day 4.

Side Effects: Common side effects may include:

  • Drowsiness  
  • Dizziness
  • Dry mouth  
  • Increased appetite
  • Weight gain  
  • Constipation  
  • Increased cholesterol and triglyceride levels

Less common but potentially serious side effects can occur. It is important to contact your doctor immediately if you experience any unusual or severe side effects.

Precautions:

  • Inform your doctor about any pre-existing medical conditions.
  • Use caution when driving or operating machinery.
  • Avoid alcohol while taking Tiapine XR.  
  • Inform your doctor if you are pregnant or breastfeeding.

Contraindications:Tiapine XR is contraindicated in patients with known hypersensitivity to quetiapine or any of the inactive ingredients.  

Drug Interactions:Tiapine XR can interact with various medications. Inform your doctor about all the medicines you are currently taking.
