Ticamet is a combination product containing Salmeterol (a long-acting beta-2 agonist) and Fluticasone Propionate (an inhaled corticosteroid). It is indicated for the regular treatment of asthma and COPD where the use of a combination product is appropriate. Salmeterol provides long-term symptom protection, while Fluticasone Propionate improves lung function and prevents exacerbations. It belongs to the therapeutic class of long-acting selective β-adrenoceptor stimulants and respiratory corticosteroids.
How Ticamet Works (Pharmacology)
Ticamet combines two drugs with different mechanisms to address the underlying causes of airway disease:
- Salmeterol: As a long-acting (12-hour) selective beta-2-adrenoceptor agonist, it causes bronchodilation by relaxing the smooth muscles in the airways. This provides sustained relief from symptoms like wheezing and shortness of breath.
- Fluticasone Propionate: This is a potent inhaled corticosteroid with anti-inflammatory action in the lungs. It reduces inflammation, leading to a decrease in asthma symptoms and a prevention of exacerbations, without the systemic adverse effects of oral corticosteroids at usual doses.
Key Indications & Benefits
Ticamet is indicated for:
- Asthma:
- In patients not adequately controlled with inhaled corticosteroids and a short-acting beta-2 agonist.
- In patients already adequately controlled on both an inhaled corticosteroid and a long-acting beta-2 agonist.
- COPD (Chronic Obstructive Pulmonary Disease).
Dosage & Administration
The dosage varies based on the formulation (Inhaler, Inhalation Capsule, or Maxhaler) and the patient's age and condition. Dosage is generally administered twice daily (morning and evening, approximately 12 hours apart). Always consult a registered physician for specific dosage instructions.
- Inhaler (Adults & Adolescents >12): 2 puffs of varying strengths (e.g., 25/50, 25/125, or 25/250 mcg) twice daily.
- Inhaler (Children 4-12 years): 2 puffs of 25/50 mcg twice daily.

- Acute Symptoms: Ticamet is not for the relief of acute asthma symptoms; patients should always have a fast-acting bronchodilator (e.g., salbutamol) available.
- Paradoxical Bronchospasm: Can occur with an immediate increase in wheezing after dosing. If this happens, discontinue the product immediately and seek medical advice.
- Pneumonia in COPD: There is an increased risk of pneumonia in patients with COPD using this type of inhaler.
- Corticosteroid Effects: Long-term use of high doses may lead to systemic effects, such as adrenal suppression, growth retardation in children, and decreased bone mineral density. It is important to use the lowest effective dose.
- Beta-Blockers: Avoid using non-selective and selective beta-blockers in asthma patients unless absolutely necessary.
Drug Interactions:
- Strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir): Co-administration may increase systemic exposure to Fluticasone Propionate.
Overdose Effects
- Salmeterol Overdose: Symptoms include seizures, angina, tachycardia, tremors, and hypokalemia. Treatment involves discontinuing the drug and using appropriate beta-blocking agents with caution.
- Fluticasone Propionate Overdose: Acute overdose can cause temporary adrenal suppression, which does not require emergency action. Chronic overdose may lead to more persistent adrenal suppression, requiring monitoring.
