Topirva 25

Presentation: Each tablet contains Topiramate 25 mg.

Indications: Topirva is indicated for:

• Monotherapy in Epilepsy: In adults and children aged 6 years and above with newly diagnosed epilepsy who have generalized tonic-clonic seizures or partial seizures with or without secondary generalization.
• Adjunctive Therapy in Epilepsy: In adults and children aged 2 years and above who are inadequately controlled on conventional first-line antiepileptic drugs for partial seizures with or without secondarygeneralization; seizures associated with Lennox-Gastaut Syndrome; and primary generalized tonic-clonic seizures. 
• Migraine Prophylaxis: In adults for the prevention of migraine headache.

Dosage and Administration: The dosage is individualized and determined by the physician. To minimize side effects, the initial dose is usually low and increased gradually.

• Monotherapy in Epilepsy (Adults and Children over 16 years): Initial dose is 25 mg nightly for 1 week, increased at 1- or 2-week intervals by 25-50 mg/day, administered in two divided doses. The recommended initial target dose is 100 mg/day, with a maximum of 400 mg/day.
• Monotherapy in Epilepsy (Children 6-16 years): Initial dose is 0.5 to 1 mg/kg nightly for 1 week, increased at 1- or 2-week intervals by 0.5 to 1 mg/kg/day, administered in two divided doses. The recommended initial target dose range is 3 to 6 mg/kg/day.
• Adjunctive Therapy in Epilepsy (Adults and Children over 16 years): The minimal effective dose is 200 mg/day, usually 200-400 mg/day in two divided doses, with a maximum of 800 mg/day. Initiate at 25-50 mg/day and titrate by 25-50 mg/week.
• Adjunctive Therapy in Epilepsy (Children 2-16 years): Recommended total daily dose is approximately 5 to 9 mg/kg/day in two divided doses. Titration begins at 25 mg nightly for the first week, increased at 1- or 2-week intervals by 1 to 3 mg/kg/day.
• Migraine Prophylaxis (Adults): Initial dose is 25 mg nightly for 1 week, increased by 25 mg/day weekly. The recommended total daily dose is 100 mg/day in two divided doses. Some patients may benefit from 50 mg/day.

Administer orally, with or without food. Swallow tablets whole. Dosage adjustments may be necessary in patients with renal impairment. Antiepileptic drugs, including topiramate, should be withdrawn gradually to minimize the risk of seizures.

Side Effects: Common side effects may include paraesthesia, anorexia, weight loss, fatigue, dizziness, somnolence, nervousness, psychomotor slowing, abnormal vision, fever, nausea, abdominal³pain, and diarrhoea. Other possible side effects include difficulty with concentration, speech problems, memory problems, lack of coordination, confusion, depression, headache, and taste disturbance.

Precautions: Use with caution in patients with renal or hepatic impairment. Ensure adequate hydration to reduce the risk of kidney stones. May cause central nervous system-related adverse events, including drowsiness and cognitive dysfunction. Exercise caution while driving or operating machinery. May interact with other antiepileptic drugs, CNS depressants, and oral contraceptives. Inform the physician if pregnant or breastfeeding.

Storage: Store below 30°C in a dry place, protected from light. Keep out of reach of children.
