Topirva XR 50 Capsule

Generic Name: Topiramate

Strength: Available in 50 mg extended-release capsules.

Indications:

• Epilepsy:
  • As monotherapy for partial-onset or primary generalized tonic-clonic seizures in adults and children 6 years and older.
  • As adjunctive therapy for partial-onset seizures, primary generalized tonic-clonic seizures, and seizures associated with Lennox-Gastaut syndrome in adults and children 2 years and older.
• Migraine Prophylaxis: For the prevention of migraine headaches in adults and adolescents 12 years of age and older.  

Dosage and Administration:

• Dosage should be individualized by a physician.
• Topirva XR is taken orally once daily.
• Swallow the capsule whole; do not crush, chew, or open it.
• It can be taken with or without food.
• The starting dose and titration rate vary depending on the indication and age of the patient.
• Refer to the product leaflet or your doctor's instructions for specific dosage guidelines.

Contraindications:

• Hypersensitivity to topiramate or any of the excipients.

Side Effects:

• Common: Paresthesia (tingling), weight loss, fatigue, dizziness, somnolence (drowsiness), nervousness, difficulty with concentration/attention, speech problems, visual disturbances.
• Less Common: Memory impairment, confusion, ataxia (lack of coordination), tremor, depression, anxiety.
• Serious: Acute myopia and secondary angle closure glaucoma, oligohydrosis (decreased sweating) and hyperthermia, metabolic acidosis, hyperammonemia, kidney stones, suicidal thoughts or behavior.

Precautions:

• Use with caution in patients with a history of kidney stones, liver disease, or kidney disease.
• May cause central nervous system depression and impair the ability to drive or operate machinery.
• Avoid alcohol use while taking Topirva XR.
• Inform your doctor if you experience any changes in vision.
• Monitor for signs of metabolic acidosis.
• Ensure adequate hydration to reduce the risk of kidney stones.
• Use with caution in pregnancy and lactation; discuss potential risks and benefits with your doctor.
• Gradually withdraw the medication to minimize the risk of increased seizure frequency.  

Use in Pregnancy & Lactation:

• Pregnancy: Topiramate can cause fetal harm. It should only be used during pregnancy if the potential benefit outweighs the risk to the fetus. 
• Lactation: Topiramate is excreted in breast milk. A decision should be made whether to discontinue breastfeeding or the drug, considering the importance of the drug to the mother. 

Storage:

• Store below 30°C (86°F).
• Protect from moisture.
• Keep out of reach of children.

Packaging: Available in strips of 10 capsules.
