Indication:
Tricalm is indicated for the symptomatic treatment of all forms of parkinsonism (idiopathic, postencephalitic, and arteriosclerotic). It is also used to control extrapyramidal disorders caused by central nervous system drugs such as phenothiazines, thioxanthenes, and butyrophenones. Trihexyphenidyl belongs to a class of medications called anticholinergics, which help to reduce tremors, muscle stiffness, and other Parkinson's-like symptoms by blocking the action of acetylcholine in the brain.
Dosage and Administration:
The initial dose for parkinsonism is usually 1 mg on the first day, given orally. The dose may then be increased by 2 mg increments at intervals of 3 to 5 days, until a total daily dose of 6 to 10 mg is reached. The total daily dose is usually administered in 3 or 4 divided doses at mealtimes and at bedtime.
For drug-induced extrapyramidal disorders, the usual dose is 1 mg once daily, which may be gradually increased until the symptoms are controlled. In some cases, doses of 5 to 15 mg per day may be required.
Postencephalitic patients may require higher doses, typically 12 to 15 mg per day.
The dosage should be individualized based on the patient's response and tolerance. It is recommended to start with a low dose and increase gradually to minimize side effects.
Tricalm tablets should be swallowed whole with water. It is important to follow your doctor's instructions carefully regarding the dosage and timing of administration.
Side Effects:
Common side effects include dry mouth, blurred vision, dizziness, mild nausea or nervousness. These side effects may diminish as therapy continues.
More serious side effects can include constipation, difficulty passing urine, rapid heartbeat, confusion, hallucinations, and increased pressure inside the eye (intraocular pressure). If you experience any of these serious side effects, you should contact your doctor immediately.
Precautions:
Tricalm should be used with caution in patients with glaucoma (especially narrow-angle glaucoma), prostatic hypertrophy, or obstruction of the bladder neck. It should also be used cautiously in patients with heart disease, liver or kidney disorders, or a history of drug abuse.
Elderly patients may be more susceptible to the side effects of this medication and may require lower doses.
The use of Tricalm during pregnancy and breastfeeding should be discussed with a doctor, as it may have potential risks to the fetus or infant.
Drug Interactions:
Tricalm can interact with other medications, including other anticholinergics, antihistamines, certain antidepressants, and some medications used to treat gastrointestinal disorders. It is important to inform your doctor about all the medications you are currently taking, including over-the-counter drugsand herbal supplements, to avoid potentialdrug interactions.
