Trileptal 600

Presentation:Each film-coated tablet contains 600 mg of oxcarbazepine. 

Indications: Trileptal (oxcarbazepine) is indicated for the treatment of:  

  • Partial seizures with or without secondarily generalized tonic-clonic seizures in adults and children aged6 years and above as monotherapy or adjunctive therapy.
  • Monotherapy in the treatment of partial seizures in children aged 4-16 years.  
  • Adjunctive therapy in the treatment of partial seizures in children aged 2-16 years.  

Dosage and Administration: Trileptal tablets should be swallowed with liquid and can be taken with or without food. The dosage is individualized and adjusted based on clinical response.

  • Adults:
    • Monotherapy: Initial dose is 600 mg/day in two divided doses (300 mg twice daily). The dose may be increased by 300 mg/day every third day to a maintenance dose of 1200 mg/day. In some refractory patients, doses up to 2400 mg/day may be used.
    • Adjunctive Therapy: Initial dose is 600 mg/day in two divided doses (300 mg twice daily). The dose may be increased by a maximum of 600 mg/day at approximately weekly intervals. The recommended maintenance dose is 1200 mg/day. Doses up to 2400 mg/day have been used.
  • Children (Aged 2-16 years): The starting dose is 8-10 mg/kg/day given in two divided doses, not to exceed 600 mg/day. The target maintenance dose is weight-dependent and is usually reached over two weeks in adjunctive therapy.  
  • Renal Impairment:In patients with severe renal impairment (creatinine clearance <30 mL/min), the initial dose should be reduced by half.Dosage should be increased slowly to achieve the desired clinical response.
  • Hepatic Impairment:No dosage adjustment is usually required for mild to moderate hepatic impairment.Caution is advised in severe hepatic impairment.

Side Effects: Very common side effects include dizziness, somnolence (drowsiness), diplopia (double vision), fatigue, and nausea. Common side effects include vomiting, headache, ataxia (uncoordinated movements), abnormal vision, abdominal pain, tremor, dyspepsia (indigestion), abnormal gait, and balance disorder.Less common side effects include hyponatremia (low sodium levels), increased hepatic enzymes, and rash. Rare but serious side effects include severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis), angioedema, and anaphylaxis.  

Precautions:

  • Trileptal is contraindicated in patients with known hypersensitivity to oxcarbazepine or eslicarbazepine acetate.
  • Use with caution in patients with a history of hypersensitivity reactions to carbamazepine, as cross-sensitivity can occur.  
  • Caution is advised in patients with renal impairment and hyponatremia. Monitor sodium levels, especially in elderly patients and those taking diuretics.
  • May decrease the effectiveness of hormonal contraceptives; alternative non-hormonal contraception is recommended.  
  • Patients with the HLA-B*1502 allele may be at increased risk of serious skin reactions, particularly those of Asian ancestry; genetic testing may be considered before initiating treatment in these individuals.  
  • May cause central nervous system depressant effects; caution patients about activities requiring mental alertness, such as driving or operating machinery.  
  • Monitor for signs and symptoms of suicidal ideation and behavior.
  • Use during pregnancy should only occur if the potential benefit outweighs the potential risk to the fetus. Breastfeeding is not recommended during treatment with Trileptal.

Storage: Store below 30°C, protected from moisture. Keep out of reach of children.
