Uromax (Tamsulosin): A Selective Alpha1 Adrenoceptor Blocker for BPH

Uromax contains Tamsulosin Hydrochloride, a selective alpha1 adrenoceptor blocking agent. It is indicated for the treatment of the functional symptoms of benign prostatic hyperplasia (BPH). It works by relaxing the smooth muscles in the prostate and bladder neck to improve urine flow and reduce symptoms. Uromax belongs to the therapeutic class of BPH/Urinary retention/Urinary incontinence drugs.


How Uromax Works (Pharmacology)

Tamsulosin's mechanism of action is to relax the smooth muscles in the prostate:

  • Selective Alpha1 Adrenoceptor Blockade: It selectively blocks alpha1A adrenoceptors, which are primarily found in the human prostate.
  • Smooth Muscle Relaxation: The blockade of these receptors causes the smooth muscles in the bladder neck and prostate to relax.
  • Improved Urine Flow: This relaxation reduces the obstruction of urine flow, leading to an improvement in urine flow rate and a reduction in the symptoms of BPH.
  • Metabolism: It is extensively metabolized in the liver, and less than 10% of the dose is excreted in an unchanged form.

Uromax is indicated for:

  • Treatment of functional symptoms of Benign Prostatic Hyperplasia (BPH).

Dosage & Administration

The recommended dose is one capsule daily, taken after a meal at night. The capsule should be swallowed whole with a glass of water while standing or sitting. It should not be crunched or chewed, as this can affect the medication's release. Always consult a registered physician for specific dosage instructions.

  • Initial dose: 0.4 mg (one capsule) once daily.
  • Increased dose: If necessary, the dose may be increased to 0.8 mg (two capsules) once daily after 2 to 4 weeks.
  • Interrupted therapy: If therapy is discontinued for several days, it should be restarted at the 0.4 mg dose.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before taking Uromax.

Contraindications:

  • Hypersensitivity to Tamsulosin.
  • History of orthostatic hypotension.
  • Severe hepatic insufficiency.

Side Effects:

  • Common: Dizziness, abnormal ejaculation.
  • Less common: Headache, asthenia (weakness), postural hypotension, and palpitations.

Pregnancy & Lactation:

  • Uromax is not recommended for use in pregnancy and lactation.

Precautions & Warnings:

  • Orthostatic Hypotension: A reduction in blood pressure can occur, especially upon standing, which may lead to dizziness or fainting (syncope). Patients should be advised to sit or lie down at the first signs of dizziness until symptoms resolve.
  • Driving and Operating Machinery: Due to the risk of dizziness, patients should be cautious when driving or operating machinery.
  • Pre-treatment Examination: Before starting therapy, patients should be examined to rule out other conditions that may cause BPH-like symptoms. A digital rectal exam and PSA test should be performed initially and at regular intervals thereafter.
  • Micturition Syncope: Use in patients with a history of fainting during urination is not advised.
  • Overdose: An overdose is likely to cause acute hypotension. Treatment is supportive, involving lying the patient down and, if necessary, using fluid expanders or vasopressors.

Drug Interactions:

  • Other alpha1-adrenoceptor antagonists: Concurrent use may lead to hypotensive effects.
  • Cimetidine: May increase plasma levels of Tamsulosin.
  • Furosemide: May decrease plasma levels of Tamsulosin.
  • Diclofenac and Warfarin: May increase the elimination rate of Tamsulosin.

Storage Conditions

Store in a cool and dry place, below 30°C, and protected from light.
