Vcand (Voriconazole): A Triazole Antifungal

 

Vcand contains Voriconazole, a triazole antifungal medicine. It is indicated for the treatment of serious fungal infections in patients 12 years of age and older. Voriconazole works by inhibiting the synthesis of ergosterol, a crucial component of fungal cell membranes. It is effective against a variety of infections, including invasive aspergillosis, candidemia, and infections caused by Scedosporium and Fusarium species.


 

How Vcand Works (Pharmacology)

 

Voriconazole inhibits fungal growth by disrupting their cell membrane synthesis:

  • Ergosterol Synthesis Inhibition: It binds to and inhibits the enzyme CYP450-dependent 14-alpha sterol demethylase.
  • Depletes Ergosterol: This inhibition leads to a depletion of ergosterol, which is a vital component of the fungal cell membrane.
  • Damages Cell Membrane: The loss of ergosterol results in structural and functional damage to the fungal cell membrane, ultimately leading to the death of the fungal cell.

Dosage & Administration

  • Oral (Adults ≥40 kg):
    • Loading dose: 400 mg every 12 hours for the first 24 hours.
    • Maintenance dose: 200 mg twice daily.
  • Oral (Adults <40 kg):

    • Maintenance dose: 100 mg twice daily.
  • Injection: Loading and maintenance doses are weight-based (mg/kg) and administered intravenously every 12 hours.

Contraindications:

  • Known hypersensitivity to Voriconazole or any components of the drug.
  • Co-administration with:
    • Terfenadine, astemizole, cisapride, pimozide, or quinidine.

    • Rifampin, carbamazepine, long-acting barbiturates, efavirenz, ritonavir, rifabutin, ergot alkaloids, and St. John's Wort.

Side Effects:
