Vildaglip-M 

Presentation: Each tablet of Vildaglip-M contains a fixed-dose combination of Vildagliptin and Metformin Hydrochloride. The available strengths may vary (e.g., Vildagliptin 50mg/Metformin 500mg, Vildagliptin 50mg/Metformin 850mg, Vildagliptin 50mg/Metformin 1000mg). It is available in strips.

Indications: Vildaglip-M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus:

• In patients inadequately controlled on metformin alone.
• In patients already treated with vildagliptin and metformin as separate tablets.
• May be used in combination with other glucose-lowering medications, including insulin, when adequate glycemic control is not achieved.

Dosage and Administration: The dosage of Vildaglip-M should be individualized based on the patient's current regimen and glycemic control, not exceeding the maximum daily dose of vildagliptin 100 mg and metformin 2000 mg. It is generally taken twice daily with meals to reduce gastrointestinal side effects associated with metformin.

• Patients already treated with metformin may be switched to Vildaglip-M containing the same total daily dose of metformin, with vildagliptin 50 mg twice daily.
• Patients already treated with vildagliptin may be switched to Vildaglip-M containing metformin at a suitable dose, with vildagliptin 50 mg twice daily.
• For patients switching from separate tablets of vildagliptin and metformin, the starting dose of Vildaglip-M should be based on the doses of each component they were already taking.
• Swallow tablets whole with water; do not crush or chew.

Side Effects: Common side effects may include nausea, vomiting, diarrhea, headache, and dizziness. Hypoglycemia may occur when Vildaglip-M is used in combination with insulin or sulfonylureas. Metformin may cause gastrointestinal disturbances. Less common side effects may include fatigue, constipation, and upper respiratory tract infections.

Precautions: Vildaglip-M is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis, including diabetic ketoacidosis, and known hypersensitivity to vildagliptin or metformin. It is not recommended in patients with moderate renal impairment (eGFR 30-59 mL/min/1.73 m²) unless the benefit is considered to outweigh the risk, and renal function should be closely monitored. Caution is advised in patients with hepatic impairment or heart failure. Avoid excessive alcohol intake. Monitor renal function prior to initiation and periodically thereafter.

Storage: Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
