Indications

Coenzyme Q10 is indicated in the following conditions:

Female Infertility: Poor ovarian reserve and Polycystic ovary syndrome (PCOS).

Male Infertility: Oligospermia, asthenozoospermia, and general male factor infertility.

Neurological Disorders: Myopathy, Ataxia, Genetic Neuromuscular Disease, Migraine, Muscular Dystrophy, Parkinson's Disease, and Alzheimer's Disease.

Cardiovascular Disease: Congestive Heart Failure (CHF), Hypertension, Angina, Cardiomyopathy, and Heart Protection During Surgery.

Other Conditions: Periodontal Gum Disease, Diabetes, Exercise Performance, Cancer, Asthma, and Thyroid Disorder.

Pharmacology

The USP refers to Coenzyme Q10 as Ubidecarenone, which is a fat-soluble vitamin-like substance. It is present in every cell of the body and acts as a vital intermediate of the electron transport system in the mitochondrial membrane for cellular respiration and ATP production. The highest requirement of Ubidecarenone is found in cells and tissues that are metabolically active, such as the heart, immune system, skeletal muscle, and gingiva, making them the most susceptible to Ubidecarenone deficiency. The production of Ubidecarenone slows down with advancing age, contributing to the manifestation of aging.

As Ubidecarenone is a lipophilic substance, it is absorbed less on an empty stomach and absorbed in greater amounts when taken with food high in lipid content. After oral administration, Ubidecarenone is absorbed from the small intestine into the lymphatic system and then taken up by the liver, where it is incorporated into lipoproteins, released into the blood circulation, and concentrated in tissues.

Ubidecarenone is distributed to various tissues of the body and can cross into the brain. The concentration varies from tissue to tissue, with high concentrations found in tissues with high metabolic activity and energy demands like the heart, muscle, liver, kidney, and brain. Ubidecarenone is metabolized in all tissues of the body. Approximately 60% of an oral dose of Ubidecarenone is excreted in the feces, and its main elimination occurs via bile.

Dosage & Administration

Co-enzyme Q10 Deficiency: 150 mg daily.

Mitochondrial Disorders: 150 mg to 160 mg, or 2 mg per kg body weight per day.

Heart Failure in Adults: 100 mg per day divided into 2 or 3 doses.

Recent Myocardial Infarction: 120 mg daily in 2 divided doses.

High Blood Pressure: 120 mg to 200 mg per day divided into 2 doses.

Isolated Systolic Hypertension: 60 mg twice daily.

Preventing Migraine Headache: 100 mg three times daily.

Parkinson's Disease: 300 mg, 600 mg, 1200 mg, or 2400 mg per day in 3 to 4 divided doses.

Infertility in Men: 200 mg to 300 mg per day.

Muscular Dystrophy: 100 mg per day.

Dividing the total daily dose into smaller amounts taken two or three times a day instead of a single large dose can help reduce side effects.

Interaction

Statins: Statin drugs reduce the endogenous synthesis of Coenzyme Q10 in the body. Supplementation may increase Coenzyme Q10 levels without adversely affecting the efficacy of statin drugs.

Warfarin: May reduce the effectiveness of warfarin.

Beta-blockers: Beta-blockers (particularly propranolol) have been reported to inhibit some Coenzyme Q10-dependent enzymes.

Antidiabetic Medications: May improve glycemic control in some type 2 diabetic patients. If this occurs, antidiabetic medication dosages may require adjustment.

Doxorubicin: May help ameliorate the cardiotoxicity caused by doxorubicin.

Contraindications

Coenzyme Q10 is contraindicated in patients who have hypersensitivity to the active substance or to any of its excipients. It is also contraindicated in patients scheduled for surgery within the next two weeks.

Side Effects

Coenzyme Q10 is safe for most adults. It can cause mild side effects including stomach upset, loss of appetite, nausea, vomiting, and diarrhea. It may also cause allergic skin rashes in some individuals.

Pregnancy & Lactation

Coenzyme Q10 should not be used during pregnancy and breastfeeding.

Precautions & Warnings

Coenzyme Q10 may lower blood sugar levels, so caution should be exercised in patients with diabetes or hypoglycemia. Coenzyme Q10 may also decrease blood pressure, so caution is advised in patients with low blood pressure or those taking antihypertensive medications.
