Indications

For patients 12 years and older treating:

• Invasive aspergillosis, candidemia (non-neutropenic), and deep tissue Candida infections.
• Esophageal candidiasis.
• Infections caused by Scedosporium apiospermum and Fusarium species.
• Patients refractory to or intolerant of other therapies.

Mechanism of Action

Voriconazole inhibits fungal CYP450-dependent 14-alpha sterol demethylase, disrupting ergosterol synthesis essential for cell membrane integrity.

Dosage & Administration

Note: Take oral forms at least 1 hour before or after meals. Always follow a registered physician's guidance.

Oral Dosing:

• ≥ 40 kg: Loading dose of 400 mg (or 10 mL) every 12h for 24h → Maintenance dose of 200 mg (or 5 mL) twice daily.
• < 40 kg: Loading dose of 200 mg (or 5 mL) every 12h for 24h → Maintenance dose of 100 mg (or 2.5 mL) twice daily.

Intravenous (IV) Dosing:

• Loading dose (all indications): 6 mg/kg every 12h for 24h.
• Maintenance dose: 3–4 mg/kg every 12h (depending on indication).

Key Safety & Interactions

• Contraindications: Hypersensitivity; co-administration with CYP3A4/2C9/2C19 substrates or strong inducers.
• Common Side Effects: Abdominal pain, anemia, blurred vision, headache, chest pain, nausea, diarrhea.
• Precautions: Treatments >180 days require benefit-risk assessment due to risk of skin carcinoma. Use in pregnancy only if potential benefits outweigh risks.

Preparation & Storage

• Suspension: Reconstitute with 25 mL boiled, cooled water. Shake well.
• Storage: Unreconstituted powder at 2°C–8°C. Reconstituted suspension at 15°C–30°C (discard after 14 days). Tablets below 25°C, protected from light.
