Vono (Vonoprazan): A Potassium-Competitive Acid Blocker (P-CAB)
Vono contains Vonoprazan, a potassium-competitive acid blocker (P-CAB). It is indicated for the treatment and prevention of various gastrointestinal conditions, including gastric ulcers, duodenal ulcers, and reflux esophagitis. It is also used as an adjunct therapy for the eradication of Helicobacter pylori. Vono belongs to the therapeutic class of Potassium competitive acid blockers.
How Vono Works (Pharmacology)
Vonoprazan's mechanism of action is to block the production of gastric acid:
- Inhibits H+, K+-ATPase: It works by inhibiting the H+, K+-ATPase enzyme in a reversible and potassium-competitive manner. This enzyme is also known as the "proton pump" and is responsible for the final step of acid secretion in the stomach.
- Does Not Require Activation: Unlike proton pump inhibitors (PPIs), Vonoprazan does not require activation by acid to be effective.
- Strong and Lasting Inhibition: Due to its strong basicity, Vonoprazan stays at the acid production site for a long time, providing a strong and lasting inhibitory effect on gastric acid production.
- Protects Mucosa: It exerts a strong inhibitory effect on the formation of mucosal damage in the upper gastrointestinal tract.
Key Indications & Benefits
Vono is indicated for:
- Treatment of gastric ulcer, duodenal ulcer, and reflux esophagitis.
- Prevention of recurrence of gastric or duodenal ulcers during low-dose aspirin or NSAID administration.
- Adjunct therapy for Helicobacter pylori eradication in conditions such as gastric ulcer, duodenal ulcer, and other related conditions.
Dosage & Administration
The dosage of Vono varies depending on the condition being treated and is taken orally once daily. It can be taken without regard to food. Always consult a registered physician for specific dosage instructions.
- Gastric ulcer: 20 mg once daily for 8 weeks.
- Duodenal ulcer: 20 mg once daily for 6 weeks.
- Reflux esophagitis: 20 mg once daily for 4 to 8 weeks.
- Maintenance therapy for reflux esophagitis: 10 mg once daily, may be increased to 20 mg.
- Prevention of ulcer recurrence (with aspirin/NSAIDs): 10 mg once daily.
- H. pylori eradication: 20 mg Vono twice daily for 7 days, in combination with two antibiotics (e.g., amoxicillin and clarithromycin or metronidazole).

Contraindications:
- Hypersensitivity to Vonoprazan or any of its components.
- Patients taking certain medications, including atazanavir sulphate, nelfinavir, or rilpivirine hydrochloride.
Side Effects:
- Common: Diarrhea, constipation, rash, nausea, and abdominal distension.
- Other potential side effects: Liver enzyme increases (e.g., AST, ALT, GGT), edema, and eosinophilia.
- Rare/Serious: Hypersensitivity reactions, including anaphylactic shock.
Pregnancy & Lactation:
- Pregnancy: It should be used in pregnant women only if the potential benefit outweighs the possible risks.
- Lactation: It is advisable to avoid administering Vono to nursing mothers. If it is indispensable, nursing should be discontinued.
Precautions & Warnings:
- Long-term Treatment: For long-term use, close observation through endoscopy is recommended.
- Maintenance Therapy: Maintenance therapy should only be given to patients with a history of recurrence. Dose reduction or discontinuation should be considered if the condition is stable.
- Renal and Hepatic Impairment: Vono should be used with caution in elderly patients and in those with mild to moderate renal or hepatic impairment. Its use should be avoided in patients with severe renal or moderate to severe hepatic impairment.
- Children: The safety and efficacy of Vono have not been established in children and adolescents under 18 years of age.
Overdose Effects
- There is no reported experience of overdose with Vono.
- Treatment for an overdose should be symptomatic and supportive. Vonoprazan cannot be removed by hemodialysis.
