Wella Tablet - (12.5mg + 5mg)  

Based on the search results, "Wella Tablet" manufactured by ACI Limited, contains a combination of two active ingredients:

  • Amitriptyline Hydrochloride BP equivalent to Amitriptyline 12.5 mg
  • Chlordiazepoxide BP 5 mg  

Indications:

Wella Tablet is indicated for the treatment of patients with moderate to severe depression associated with moderate to severe anxiety.Symptoms likely to respond within the first week of treatment include:  

  • Insomnia  
  • Feelings of guilt or worthlessness  
  • Agitation  
  • Psychic and somatic anxiety  
  • Suicidal ideation  
  • Anorexia  

Pharmacology:

  • Amitriptylineis a tricyclic antidepressant that works by inhibiting the reuptake of norepinephrine and serotonin in thebrain, increasing their synaptic concentration. This is responsible for its antidepressant activity.   
  • Chlordiazepoxideis a benzodiazepine with anxiolytic properties. It enhances the effect of the inhibitory neurotransmitter GABA in the central nervous system, leading to membrane hyperpolarization and a sedative and anxiolytic effect. It also has some muscle relaxant and anticonvulsant activity.  

Dosage and Administration:

The optimal dosage varies with the severity of symptoms and the individual patient's response. The usual initial dosage is 3 or 4 tablets daily in divided doses. The larger portion of the total daily dose may be taken at bedtime. Once a satisfactory response is obtained, the dosage should be reduced to the smallest amount needed to maintain remission.   

Contraindications:

Wella Tablet is contraindicated in patients with:

  • Hypersensitivity to either benzodiazepines or tricyclic antidepressants or any components of the product.  
  • The acute recovery phase following myocardial infarction.
  • Should not be given concomitantly with a monoamine oxidase inhibitor (MAOI).

Precautions:

  • Use with caution in patients with a history of seizures, urinary retention, angle-closure glaucoma, and cardiovascular disorders.
  • Close supervision is required when Wella is given to hyperthyroid patients or those on thyroid medication.  
  • The usual precautions should be observed when treatingpatients with impaired renal or hepatic function.  
  • Antidepressants may increase the risk of suicidal thoughts and behaviors in pediatric and young adult patients. Monitor all patients closely for clinical worsening and emergence of suicidal thoughts and behaviors.
  • Use with caution in elderly or debilitated patients due to the risk of ataxia, over-sedation, confusion, or anticholinergic effects.  
  • The use of benzodiazepines, including chlordiazepoxide, exposes users to the risks of abuse, misuse, and addiction, which can lead to overdose or death. Continued use may lead to clinically significant physical dependence.   

Side Effects:

The most common side effects may include:

  • Drowsiness
  • Dry mouth
  • Constipation  
  • Blurred vision
  • Dizziness  
  • Bloating
  • Anorexia
  • Fatigue
  • Weakness
  • Restlessness
  • Lethargy

Less common side effects may include vivid dreams, impotence, tremor, confusion, and nasal congestion.  

Drug Interactions:

Because of its Amitriptyline component, Wella may block the antihypertensive action of guanethidine or compounds with a similar mechanismof action. It may enhance the response to alcohol, barbiturates, and other CNS depressants. Simultaneous administration of MAO inhibitors may cause hypertensive crises.  

Pregnancy and Lactation:

Safe use of this preparation during pregnancy and lactation has not been established. Patients should inform their physicians if they become pregnant or intend to become pregnant during therapy. It is not known whether this drug is excreted in human milk, and nursing is generally not recommended while a patient is on this drug.

Storage:

Store as directed on the product packaging, usually in a cool, dry place away from light. Keep out of reach of children.
