Xelmet XR 500

Presentation:Extended-release tablets containing 500 mg of metformin hydrochloride.

Indications:Xelmet XR 500 is indicated for the treatment of type 2 diabetes mellitus, particularly in overweight patients, when dietary management and exercise alone do not result in adequate glycemic control.In adults, it can be used as monotherapy or in combination with other oral antidiabetic agents or with insulin.In children from 10 years of age and adolescents,it can be used as monotherapy or in combination with insulin.  

Dosage and Administration: The usual starting dose of Metformin XR (including Xelmet XR) in adults is 500 mg once daily with the evening meal. The dose may be increased in increments of 500 mg weekly, up to a maximum of 2000 mg once daily with the evening meal, or alternatively increased to 1000 mg twice daily taken with meals. The dosage should be individualized based on effectiveness and tolerability, without exceeding the maximum recommended daily dose. Xelmet XR tablets should be swallowed whole and never crushed, cut, or chewed.Patients receiving immediate-release metformin tablets may be switched to Xelmet XR at the same total daily dose, up to a maximum of 2000 mg daily.

Side Effects: Very common side effects include gastrointestinal disorders such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite. These usually occur at the start of therapy and resolve spontaneously in most cases. A slow increase in dose may improve gastrointestinal tolerability. A rare but serious side effect is lactic acidosis. Other possible side effects include taste disturbance and decreased vitamin B12 absorption with long-term use.

Precautions:

  • Inform your doctor about any pre-existing medical conditions, especially kidney disease (eGFR < 30 mL/min is a contraindication), liver disease, heart failure, or any serious acute medical illness that can affect kidney function. 
  • Xelmet XR is contraindicated in patients with acute metabolic acidosis (such as lactic acidosis or diabetic ketoacidosis). 
  • Temporary discontinuation of Xelmet XR may be necessary before surgical procedures or radiological studies involving iodinated contrast agents.
  • Excessive alcohol intake should be avoided while taking Xelmet XR, as it can increase the risk of lactic acidosis. 
  • Inform your doctor if you are pregnant or breastfeeding. Management of diabetes during pregnancy should ideally be with insulin. Metformin is secreted in human breast milk, and a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importanceof the drug to the mother. 
  • Beaware of potential drug interactions and inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
  • Regular monitoring of renal function is advised. Annual estimation of vitamin B12 levels may be necessary during long-term use.

Storage: Store below 30°C, protected from moisture. Keep out of the reach of children.
