Zif-A is a tablet containing a combination of:

  • Ferrous Ascorbate: Equivalent to 48 mg of elemental iron.
  • Folic Acid: 0.50 mg (500 mcg).
  • Zinc Sulfate Monohydrate: Equivalent to 22.5 mg of elemental zinc.

It is manufactured by Square Pharmaceuticals Ltd. in Bangladesh.

Indications:

Zif-A is indicated for the prophylaxis (prevention) of deficiencies of iron, folic acid, and zinc, especially:

  • During pregnancy.
  • In cases of anemia due to iron deficiency.
  • When the diet is inadequate in these nutrients.

Dosage and Administration:

  • Adults and Elderly: Usually one tablet daily, before or after a meal (absorption is food-independent), or as directed by a physician.
  • In more severe cases, two tablets daily may be prescribed.
  • Pediatric patients: Safety and effectiveness in children have not been established.

Pharmacology:

  • Ferrous Ascorbate: A stable chelate of iron and ascorbic acid (Vitamin C), which enhances iron absorption. Iron is essential for hemoglobin production and oxygen transport.
  • Folic Acid: A B vitamin crucial for red blood cell formation, DNA synthesis, and cell division. It is vital during pregnancy for the development of the baby's brain and spinal cord.
  • Zinc: An essential mineral involved in numerous bodily functions, including immune function, wound healing, and the function of many enzymes.

Side Effects:

The most common side effects are usually mild and transient and can include:

  • Epigastric pain (upper abdominal pain)
  • Heartburn
  • Nausea
  • Vomiting
  • Diarrhea
  • Constipation

Allergic sensitization has also been reported with folic acid.

Precautions:

  • Care should be taken in patients who may develop iron overload (e.g., those with hemochromatosis, hemolytic anemia, or red cell aplasia).  
  • Failure to respond to treatment may indicate other causes of anemia that need investigation.   
  • In patients with renal failure, there may be a risk of zinc accumulation.

Contraindications:

Zif-A is contraindicated in patients with:

  • Known hypersensitivity to any of its ingredients.
  • Hemolytic anemia.
  • Conditions with increased body iron content.

Pregnancy and Lactation:

  • Use in the first trimester of pregnancy should be avoided unless there is definite evidence of iron deficiency.
  • Prophylaxis of iron deficiency is justified during the remainder of pregnancy, especially when zinc supplementation is also required.
  • During breastfeeding, it is advised to consult a physician before taking or continuing the drug.

Drug Interactions:

  • Iron can chelate with tetracycline antibiotics, potentially reducing the absorption of both. Avoid taking Zif-A within two hours of tetracycline antibiotics.
  • The absorption of iron may be reduced by concurrent use of penicillamine and antacids.

Storage:

Store below 30°C and keep out of reach of children.
