Zilon 40 

Presentation: Each enteric-coated capsule contains Omeprazole BP 40 mg. Zilon 40 is a brand name marketed by Radiant Pharmaceuticals Ltd. in Bangladesh.

Indications: Zilon (Omeprazole) is a proton pump inhibitor (PPI) indicated for the treatment of acid-related disorders, including:

  • Benign gastric and duodenal ulcers
  • Prevention of relapse in gastric ulcer
  • Prevention of relapse in duodenal ulcer
  • Erosive esophagitis
  • Hypersecretory conditions (e.g., Zollinger-Ellison syndrome)
  • NSAID-associated duodenal or gastric ulcer and gastroduodenal erosions  
  • Duodenal or benign gastric ulcer associated with Helicobacter pylori   (in combination with antibiotics)
  • Gastric acid reduction during general anesthesia
  • Gastro-esophageal reflux disease (GERD)
  • Acid-related dyspepsia

Dosage and Administration: Zilon 40 capsules should be taken one hour before a meal. The capsules should be swallowed whole with water and should not be chewed or crushed.

  • Benign gastric and duodenal ulcer: 20 mg once daily for 4 weeks in duodenal ulceration or 8 weeks in gastric ulceration. In severe or recurrent cases, the dose may be increased to 40 mg daily.  
  • Prevention of relapse in gastric ulcer: 20 mg once daily, increased to 40 mg once daily if necessary.  
  • Prevention of relapse in duodenal ulcer: 20 mg once daily (range 10-40 mg daily).  
  • NSAID-associated duodenal or gastric ulcer and gastroduodenal erosions: 20 mg once daily for 4 weeks.  
  • Helicobacter pylori eradication: Omeprazole 20 mg twice daily is recommended in combination with antimicrobial agents such as Amoxicillin and Metronidazole, or Clarithromycin and Metronidazole, for one week.
  • Zollinger-Ellison syndrome: Initially 60 mg once daily, with a usual range of 20-120 mg daily. Doses above 80 mg should be administered in two divided doses.
  • Gastric acid reduction during general anesthesia: 40 mg on the preceding evening, then 40 mg 2-6 hours before surgery.
  • Gastro-esophageal reflux disease: 20 mg once daily for 4 weeks. For the treatment of GERD and maintenance of healing of erosive esophagitis in pediatric patients (1-16 years of age), the recommended daily dose is based on body weight.
  • Acid-related dyspepsia: 10-20 mg once daily for 2-4 weeks according to response.

Side Effects: Omeprazole is generally well-tolerated. The adverse events that may occur include:

  • Diarrhea
  • Headache
  • Flatulence
  • Abdominal pain
  • Constipation
  • Dizziness/vertigo

Less common side effects may include nausea and vomiting. Long-term use may potentially lead to low magnesium levels and an increased risk of bone fractures.

Precautions: Zilon (Omeprazole) is contraindicated in patients with known hypersensitivity to omeprazole or any of the components of this medication. Gastric malignancy should be ruled out before initiating therapy. Caution should be exercised in pregnant and breastfeeding women, and the daily dose in elderly patients should, as a rule, not exceed 20 mg. In patients with severe liver disease, dose adjustment may be needed.

Omeprazole can delay the elimination of diazepam, phenytoin, and warfarin. There is no evidence of interaction with theophylline, propranolol, metoprolol, lidocaine, quinidine, amoxicillin, or antacids. The absorption of Omeprazole is not affected by alcohol or food, although it is recommended to take it before meals. Concomitant use with clopidogrel should be avoided as omeprazole may reduce its pharmacological activity.

Use in Pregnancy and Lactation: The safety of Omeprazole in pregnancy has not been established. It should not be administered to pregnant women unless the expected benefits outweigh the potential risks. It is not known if Omeprazole is secreted in human milk and should not be given to nursing mothers unless its use is considered essential.

Storage: Keep away from light and store below 30°C. Keep out of the reach of children.
